Administering Medications

If you require the school to administer medicine to your child, complete this Administering Medicine Form and provide a signed copy to the school office. You can also scan the signed form and send it to info@deerparkschool.org.uk.

Please note: all requests must be supported by documentary evidence that the medicine is a prescription. We do not administer over the counter, non-prescription medication (e.g. Calpol).
